The Returning Home Program is based in Southern Adelaide and provides services to mental health clients who are transitioning to the community from Glenside Hospital. Services are delivered from a recovery and rehabilitation framework, and staff work with clients and carers to ensure their well-being and quality of life is both maintained and enhanced.
As a Community Rehabilitation Worker, you will work closely with the Clinical Mental Health teams, clients and carers. In this role, you will provide case work which includes the development of individual plans, with the aim of supporting clients to live independently in the community.
